Free Radial Artery Forearm Flap for Head and Neck Reconstruction - Girish N Amlani - Bog - Jaypee Brothers Medical Publishers - Plusbog.dk

A radial forearm free flap is one way of filling a hole which is left when a cancer has been removed. It is one of the most common ways of replacing tissue in the head and neck, particularly after mouth cancers have been removed. It can be used to replace large parts of the mouth and has the advantage that when it heals it does not shrink so that hopefully speech and swallowing will not be greatly affected. For the procedure, a surgeon takes a piece of skin from the inside surface of the patient’s forearm near the wrist. The skin and fat layer in this region are removed (the flap) along with two blood vessels, one of which supplies blood to the flap (the artery) and one of which drains blood from it (the vein). The vessel which supplies blood to the flap is the artery which gives rise to the pulse at the wrist at the base of the thumb. Once the flap of skin is raised it is transferred to the head and neck and sewn into the hole created by the removal of the cancer. The blood vessels supplying and draining the flap are then joined to blood vessels in the neck under a microscope. These blood vessels then keep the flap alive while it heals into its new place. Once the flap is removed from the forearm the hole created is covered with a graft of skin. This graft of skin can be taken from one of several places. Commonly a thin piece of skin is shaved from the arm above the elbow. Alternatively, some skin will be borrowed from the stomach. This highly illustrated colour atlas is a comprehensive guide to head and neck reconstruction using free radial artery forearm flap. Beginning with an introduction to the history of the technique and its advantages and disadvantages, the book then provides step by step guidance on surgical anatomy, pre- and post-operative care, surgical procedures, and potential complications. A selection of case studies is included to assist learning. Key points - Colour atlas explaining radial forearm free flap for head and neck reconstruction - Presented as step by step guide through procedures and pre- and post-operative care - Includes case studies to assist learning - Highly illustrated with surgical images and diagrams

Support Processes in Intimate Relationships - - Bog - Oxford University Press Inc - Plusbog.dk

In the past twenty years or so, research on support processes in relationships has emerged as a distinct development in the field. Researchers have drawn from studies in the fields of communication, social support, and intimate relationships to conduct research examining support processes in relationships on micro and macro levels. Theoretical models of support processes in intimate relationships have been developed and increasingly sophisticated methodologies and data analytic techniques are being used to accumulate considerable and convincing evidence of the importance and complexity of support processes in intimate relationships. This edited book offers a broad yet coherent view of the field, showcasing novel, state-of-the-art research and theory on support processes in intimate relationships. Cutting-edge scholarly work is compiled in one accessible volume, which is designed to provoke and guide new research on social support. The book is divided into five sections designed to reflect emerging themes in the literature on support processes and intimate relationships. "Getting What One Wants: Perceived Support in Intimate Relationships" highlights the importance of offering support that is consistent with the needs of the recipient. "Providing What Partners Need: Interpersonal Aspects of Support" focuses on the importance of empathic understanding, validation of support seekers'' needs, attachment styles, and the emotional context for effective support provision. "Complexities of Support Processes in Individual and Couple Well Being" highlights the complex nature of support, presenting research on the effects of partner support on coping with stress, differential responses to daily support, and the importance of providing support for positive events. "Support in the Context of Health-related Problems and Behaviors" is comprised of chapters describing the effects of support on health, illness, and injury. Finally, "Culture and Gender" presents research that explores the role of gender and culture in support processes in couples.
