In a break with the past, this Fourth Edition de-emphasizes the design and physical operating basis of the instrumentation, including the electron sources, lenses, detectors, etc. In the modern SEM, many of the low level instrument parameters are now controlled and optimized by the microscope’s software, and user access is restricted. Although the software control system provides efficient and reproducible microscopy and microanalysis, the user must understand the parameter space wherein choices are made to achieve effective and meaningful microscopy, microanalysis, and micro-crystallography. Therefore, special emphasis is placed on beam energy, beam current, electron detector characteristics and controls, and ancillary techniques such as energy dispersive x-ray spectrometry (EDS) and electron backscatter diffraction (EBSD). With technological advance, the difficulties faced by decision makers and researchers become even more complex and hence more difficult to understand and manage. Traditional approaches have their limitations, particularly when dealing with issues that span many fields of endeavor. Fortunately there has emerged, particularly over the past four decades, the discipline of systems science, which provides a framework for dealing with such complexity. This book gives an account of the underlying theory of systems science and illustrates its applicability to a range of "real-world" problems. To gain an understanding of systems science and what motivates the systems scientist requires at least a reasonable degree ofliteracy and numeracy, a consequence of the interdisciplinary nature of the subject. The numerate content of this book, however, is almost entirely confined to Chapters 8 and 9. As a result, those who class themselves as nonnumerate are not continually confronted by equations that would, in some cases, prevent satisfactory comple­ tion of the text. Nevertheless, it has not been possible to exclude totally all aspects of numerate thinking from the remaining chapters. It would be useful, therefore, for those who class themselves as nonnumerate to read initially the section of Chapter 8 entitled "Using Letters Instead of Numbers. " This provides sufficient material to enable the nonnumerate reader to deal with the small amount of quantitative material outside Chapters 8 and 9.

Almost every effort in the care of patients with heart disease begins with some description of disordered physiologic performance or mor­ phologic anatomy. Since the early work of Edler and Hertz in 1954, echocardiographic methods have grown in importance and reliability for the diagnosis of many cardiac disorders. The placement of a maneuverable transducer on the tip of a modified endoscope is the result of relatively recent technologic advances. The transesophageal approach is now a reality for obtaining new information from ultra-sonic images of a beating human heart. Since images obtained by transesophageal echo cardiography are uniformly of excellent quality, it extends the diagnostic potential of echocardiography to the patient who is difficult to image from the conventional chest wall approach. More importantly, transesophageal echocardiography provides a means to acquire useful information in new situations, such as the operating room. When this imaging modality is brought to patients undergoing surgical procedures, surgeons and anesthesiologists have a ready means for assessing cardiac performance during anesthesia, directing various surgical approaches and immediately evaluating the results of surgical repair. Transesophageal echocardiographic techniques represent a major vi vii advance in the care of patients with cardiovascular disease. Never before has there been a means to acquire such important information about the heart during an operative intervention. New questions are being asked and new answers are at hand.

The color photographs and illustrations appearing in select monographs over the past decade represent a significant advance in surgical commu­ nication. Previously, black and white renderings were standard because color printing had not been perfected and its cost was prohibitive. We be­ lieve the gradual change from black and white etchings and reproductions to detailed color illustrations represents a tremendous twentieth century achievement. In our own urologic practices it became evident that the use of black and white sketches for teaching operative urology, while helpful, left too much to the surgeon''s imagination. Even the dimension of depth, produced by varying the intensity of shading, often was inadequate in demonstrat­ ing subtle surgical details in a fossa or bed. Since color photography recently has been used to demonstrate oper­ ative techniques, we climbed on the bandwagon and photographed in color many of the major operative procedures. However, the reflection of light caused by normal tissue fluid significantly reduces the clarity of photo­ graphs. Not infrequently, the crucial operative step is obscured by light reflected on a trickle of blood. Even when meticulous care is taken to ir­ rigate and clean the surgical field, the tissue sheen obscures portions of the field. Color paintings by a gifted artist are not only unique, but can be extraordinary; the vividness with which subtleties can thus be portrayed cannot be matched by photography, black and white or color.

The purpose of the World Psychiatric Association is to coordinate the activities of its Member Societies on a world-wide scale and to advance enquiry into the etiology, pathology, and treatment of mental illness. To further this purpose, the Association organizes mono- or multi thematic Regional Symposia in different parts of the world twice a year, and World Congresses dealing with all individual fields of psychiatry once every five or six years. Between these meetings the continuation of the Association''s scientific work is assured through the activities of its specialty sections, each covering an important field of psychiatry. The programs of the World Congresses reflect on the one hand the intention to present the coordinating functions of the Association and on the other to open a broad platform for a free exchange of views. Thus, the VII World Congress of Psychiatry, held in Vienna from July 11 to 16, 1983, was composed of two types of scientific events - those structured by the Association and those left to the initiative of the participants. The first type comprised Plenary Sessions, planned by the Scientific Program Committee, anq Section Symposia, organized by the WPA sections; the second embraced Free Symposia, free papers, video sessions, and poster presentations prepared by the participants. Altogether, 10 Plenary Sessions, 52 Section Symposia, and 105 Free Symposia took place, and 78 free papers and poster sessions and 10 video sessions were held.

Latinos are the fastest growing and largest minority group in the United States. In 2008, this group numbered over 47 million; by 2050, the population is expected to triple, reaching 133 million. Traditionally, Latinos have immigrated to large urban centers (e.g., New York, Los Angeles) that over long periods of time developed a complex infrastructure to receive new immigrants. Increasingly, new Spanish-speaking immigrants are moving into areas of the country previously unfamiliar to them. Although urban co-ethnic communities continue to be the destination of many newcomers, immigrants from Mexico, Central America, and South America in pursuit of low-skilled labor opportunities are settling in small towns and rural areas of the South and Midwest. This new demographic trend has resulted in the creation of the term "new growth communities," which refers to small rural areas that are now home to a small but rapidly growing Hispanic population. Unfortunately, these communities, which are now present in many states across the country (e.g., Illinois, North Carolina), lack the infrastructure necessary to meet the needs of Latino immigrants (e.g., access to health care, immigration assistance, and breaking down language barriers). The lack of an infrastructure and the lack of an established ethnic community to facilitate the assimilation of new immigrants present an ongoing challenge, especially in the area of Latino mental health. The volume focuses on dealing with systemic issues and on providing innovative ideas for development of infrastructure of services. This text will advance ways to understand and ameliorate mental health disparities both from research and experiential perspectives.
