The requirement for additional energy in the future is so daunting that we will need to use all resources. In "connecting the dots", we engage in a distinctive sort of self-expression. In the first group of essays in this volume, Fischer argues that we do not need genuine access to alterative possibilities in order to be morally responsible. Thus, the line need not branch off at crucial points (where the branches represent genuine metaphysical possibilities). In the remaining essays in the collection he demonstrates that deep control is the freedom condition on moral responsibility. In so arguing, Fischer contends that total control is too much to ask--it is a form of "metaphysical megalomania". So we do not need to "trace back" all the way to the beginning of the line (or even farther) in seeking the relevant kind of freedom or control. Additionally, he contends that various kinds of "superficial control"--such as versions of "conditional freedom" and "judgment-sensitivity" are too shallow; they don''t trace back far enough along the line. In short, Fischer argues that, in seeking the freedom that grounds moral responsibility, we need to carve out a middle ground between superficiality and excessive penetration. Deep Control is the "middle way". Fischer presents a new argument that deep control is compatible not just with causal determinism, but also causal indeterminism. It may come as something of a surprise that pain, the most prevalent symptom in clinical practice, is not always addressed specifically in health professions training. Approximately one in six Americans lives with chronic pain in addition to the millions that experience acute pain each day. Half of older adults live with chronic pain-associated conditions, and about half of all healthcare visits are initiated because of pain. Despite this, reports indicate that the vast majority of health professions schools in the United States do not teach required courses on pain, and the total amount of content pertaining to pain is a fraction of a percent of the total. Almost certainly, the lack of education in coordinated, comprehensive, compassionate care for pain-associated conditions contributed to pervasive opioid over-prescribing and the ensuing wave of addiction and deaths that swept the country in the first part of this century. This book is our response to the pain care crisis - it is designed to prepare young clinicians to assess and treat a wide variety of pain conditions in a manner that balances competence and compassion, incorporating coordinated elements of pharmacological and non-pharmacological therapies. Designed to be read during or after pre-licensure training, e.g. medical, nursing, pharmacy school, and to inspire students to learn more about painful conditions, this book is unique in its clinical focus and the level of detail that is included. This book aims to improve pain care, most especially if used alongside a formal pain care course as part of pre-licensure training, whether spread over four years or condensed into a shorter period. The focus is on enhancing shareholder value creation and the execution of strategies for the successful management of mergers. It also has a strong public-policy component in this "special" industry where successes can pay dividends and failures can cause serious problems that reach well beyond the financial services industry itself. The financial services sector is about halfway through one of the most dramatic periods of restructuring ever undergone by a major global industry. The impact of the restructuring has carried well beyond shareholders of the firms and involved into the domain of regulation and public policy as well as global competitive performance and economic growth. Financial services are a center of gravity of economic restructuring activity. M&A transactions in the financial sector comprise a surprisingly large share of the value of merger activity worldwide -- including only deals valued in excess of $100 million, during the period 1985-2000 there were approximately 233,700 M&A transactions worldwide in all industries, for a total volume of $15.8 trillion. Of this total, there were 166,200 mergers in the financial services industry (49.7%), valued at $8.5 trillion (54%). In all of restructuring frenzy, the financial sector has probably had far more than its share of strategic transactions that have failed or performed far below potential because of mistakes in basic strategy or mistakes in post-merger integration. It has also had its share of rousing successes. This book considers the key managerial issues, focusing on M&A transactions as a key tool of business strategy - "doing the right thing" to augment shareholder value. But in addition, the degree of integration required and the historic development of integration capabilities on the part of the acquiring firm, disruptions in human resources and firm leadership, cultural issues, timeliness of decision-making and interface management have co-equal importance - "doing it right."
