Artists, musicians, squatters, club owners, DJs and ravers are reclaiming the old city centre and bringing it back to life. This interregnum between two systems – the collapse of the old East Germany, the gentrification of the new Berlin – lasts only a few years. West Berliners, East Berliners and new residents from abroad join together to create music, art and fashion, to open bars and clubs and galleries, even if only for a few weeks. In the months following the fall of the Wall, there is a feeling of new beginnings and immense possibilities: life is now, and to be in the here and now feels endless. The phrase ‘temporary autonomous zone’ is circulating, it describes the idea – romantic and naive but, in the circumstances, not absurd – that, at a certain moment in history, you can actually do whatever you want. Ulrich Gutmair moved to West Berlin as a student in autumn 1989: two weeks later the Wall came down. He spent the next few years studying during the day in the West and exploring the squats, bars and techno clubs in the East at night. He fell in love with House and Techno and raved at Tresor, Elektro, Bunker and many other places that in the meantime have almost disappeared from collective memory. Ten years later he decided to write a book about that period in between, when one regime was brought down and a new one wasn’t yet established. Gays must be normal but not completely so, similar and at the same time different, in order to meet the not always conscious conditions of acceptability. Gayfriendliness has managed to dispel violence and discrimination and has accompanied the invention of less conventional lives. But, as Tissot shows, it has not yet liberated itself from the clutches of heterosexual domination which still structures our society and our ways of thinking.

Insulin resistance, defined as a reduced biological action of insulin, has emerged as a major factor in the development and progression of a number of common non-communicable diseases in man. The role of insulin resistance in the aetiology of type 2 diabetes is particularly well-established. However, insulin resistance has also come to be regarded as a key component of a broader syndrome of common metabolic defects that conspire to increase the risk of atherosclerotic coronary heart disease. The ramifications of insulin resistance now embrace many different medical specialties. The objective of this book is to summarize the current state of knowledge about insulin resistance. Section 1 (Pathophysiology of Insulin Resistance) considers the development of current concepts of insulin resistance. This is followed by a critical review of techniques for the assessment of insulin action in humans. The section concludes with an outline of current hypotheses concerning the molecular defects responsible for insulin resistance. Section 2 (Insulin Resistance in Clinical Medicine) broadens the discussion to include physiological and pathological conditions with which insulin resistance is associated; the effects of drug treatment on insulin sensitivity are also considered. Section 3 (Management of Insulin Resistance and Associated Conditions) focuses on the avoidance and treatment of insulin resistance in its clinical manifestations. A discussion of the potential benefits of non-pharmacological measures prefaces a review of the range of drugs used in the treatment of type 2 diabetes and related disorders. References are confined to key articles at the end of each section. Improved nutrition allied to increased levels of physical exercise are of crucial importance in the battle to stem the increasing incidence and prevalence of insulin resistance; however, translation of such advice into action has proved difficult in practice. Thus, other approaches have been explored. Recent years have witnessed considerable progress in the pharmacological management of insulin resistance. In particular, the thiazolidinediones (and other insulin-sensitizing drugs) appear to provide a more specific assault on insulin-resistance. Clinical experience with these agents remains relatively limited and serious hepatotoxicity has clouded the first agent in this class - troglitazone. With its introduction into the USA in 1995 and driven by the publication of the United Kingdom Prospective Diabetes Study, metformin has enjoyed a renaissance. This well-established agent is finding application for new clinical indications. In parallel, the development of anti-obesity drugs continues apace. The molecular and genetic mechanisms underlying some forms of insulin resistance are being revealed. Original intellectual concepts such as the fetal origins hypothesis are challenging traditional views about the aetiology of insulin resistance and its clinical sequelae. These exciting scientific advances notwithstanding, the evolving global epidemic of obesity and type 2 diabetes represents an enormous public health challenge. Now more than ever, research must focus on identifying solutions that are applicable on the level of entire populations. By definition, these must also be practical and cost-effective.

A unique feature of this book is that it deals with sustainable management of river sediments by explicitly incorporating sediment management in the planning, design and operation of dams, both to extend reservoir life and to minimize downstream impacts of sediment starvation. This book takes a river system perspective first, explaining how dams (and other anthropic actions such as aggregate mining) disrupt continuity of sediment transport, the consequences of that disruption, and opportunities to sustainably manage sediments, especially by passing sediments through or around dams. The book also addresses the likely consequences of climate change on reservoir operation and sediment supply and transport. It is not the first book about reservoir sediment management, nor the most comprehensive. Rather, it has more emphasis on geomorphic and ecological processes in the larger fluvial system, emphasizes the fact that sustainable sediment management techniques could be applied much more widely than they are now, and it offers guidance as to when different techniques are applicable. It also explicitly critiques the conventional economics used in dam planning, and proposes alternative formulations that explicitly account for intergenerational equity as it relates to natural river systems and infrastructure (Annandale 2013). What the Book Covers The book begins with an overview of river basin processes, including sediment transport, and effects of changed sediment supply (chapters 1-3). Chapter 4 covers upstream sediment management approaches such as check dams and soil conservation. Chapters 5-7 summarize sustainable sediment management in (and around/through) reservoirs. Chapter 8 explores sediment management in dam removals, usually the biggest issue in such projects. Chapter 9 provides a long-term perspective and explores the economics of managing reservoir capacity. Chapter 10 presents a set of case studies that compellingly illustrate the issues involved and opportunities to enhance sustainability, and Chapter 11 presents policy recommendations. The book will include simple, effective diagrams explaining fluvial geomorphic processes, human alterations, and sustainable sediment management techniques. Two examples are presented here. Figure 1, first published in Kondolf (1994), shows the zones of erosion, transport, and deposition of the ideal fluvial system as described by Schumm (1977), but with the addition of the analogy of a conveyor belt to emphasize that, viewed in geologic time, sediment is inevitably moving downstream through the zone of transport, and disruptions to this movement will have consequences. This figure has been reproduced and modified in subsequent publications by numerous authors. Figure 2, first published in Kondolf and Piégay (2010), depicts a fluvial system debouching on the coast and the effect of various anthropic actions on sediment production, transport, and deposition, and consequences for channel and coastal landforms.
