Langsam begibt er sich auf den Pfad des Verbrechens und wird im Laufe des Spiels Vollmitglied der einflussreichen, örtlichen Mafia-Familie. Die Gameplay-Mechanik in Mafia II ist in fast allen Aspekten eine Erweiterung der Ideen, die in dem acht Jahre zuvor veröffentlichten Vorgänger verwendet wurden. Der Handlungsort ist erneut eine Stadt mit einer offenen Struktur von etwa 10 Quadratmeilen - das monumentale Empire Bay, das eigentlich eine lose, virtuelle Adaption von New York ist (im Spiel treffen wir unter anderem auf so charakteristische Gebäude wie das Empire State Building oder die Brooklyn Bridge). Die Stadt ist in mehrere verschiedene Bezirke unterteilt, darunter Little Italy, Chinatown, ein Industrie- oder Hafenviertel, ein Viertel der Reichen sowie Elendsviertel). Wie im Original ist die Achse des Spiels eine ziemlich lineare Handlung, für die insgesamt etwa 10-15 Stunden Spielzeit benötigt werden. Die nachfolgenden Aufgaben sind recht abwechslungsreich und bieten unter anderem spektakuläre Scharmützel mit Mitgliedern rivalisierender Mafia-Familien, Autofahrten, bei denen wir jemandem folgen oder vor der Polizei fliehen müssen, und sogar Herausforderungen, die das Handeln im Verborgenen erfordern. Der Spieler ist jedoch nicht gezwungen, dem Handlungsverlauf zu folgen und kann sich im Laufe des Spiels auch zahlreichen Nebenhandlungen und zusätzlichen Aktivitäten widmen. Im Vergleich zum ersten Teil der Serie bietet Mafia II eine viel größere Handlungsfreiheit und die Möglichkeit, die Stadt um uns herum ungehindert zu erkunden, die ihr eigenes Leben führt und mit einer großen Anzahl nicht ganz legaler Attraktionen lockt. Beim Spielen können wir Fahrzeuge stehlen (indem wir die Fensterscheibe einschlagen oder etwas subtiler vorgehen) und aus über 50 Modellen wählen, die auf echten Marken basieren, und sie dann in der Werkstatt verbessern und modifizieren. In Empire Bay finden wir auch viele Geschäfte, in denen wir Kleidung kaufen, essen oder neue Waffen bekommen können. Unser Protagonist hat auch eine Reihe neuer Fähigkeiten erlangt und kann jetzt zum Beispiel mit mehr Schlägen kämpfen, sich hinter verschiedenen Deckungen verstecken und über Hindernisse springen. Da die Handlung des Spiels etwas später als die Ereignisse von Mafia I stattfindet, begleiten uns auch etwas andere, noch grausamere Realitäten sowie ein völlig anderer Stil der Stadt und viel schnellere Autos.
